Who Are We

Transition Homeless Project is a registered charity supporting individuals experiencing poverty, housing insecurity, homelessness, or rough sleeping. With a strong focus on prevention, we work to intervene early — long before anyone reaches crisis point.

Operating across North West London, we provide practical support, advice, and advocacy that empowers people to stabilise their lives and build sustainable futures. Our team works alongside adults and young people, offering guidance that builds confidence, strengthens resilience, and opens pathways to secure, sustainable, independent living within their communities.

Our work is rooted in dignity, compassion, and community impact. By addressing the underlying causes of poverty and homelessness, we help people move from vulnerability to stability, and from crisis to opportunity.

Our Mission

Through compassionate care and tailored services, Transition Homeless Project is dedicated to fostering relationships that make a lasting difference in the lives of those in poverty or experiencing homelessness. 

We work collaboratively with other local community groups and local councils to break the cycle of poverty and homelessness.

Why We Need Your Help

Homelessness can happen to anyone

The rising cost of living and energy prices have left many struggling to survive. In London alone:

  • Over 187,000 people are homeless (Shelter, December 2024).
  • 24.8% of homeless individuals in England and Wales are based in London.
  • People aged 25 and under make up 9% of those sleeping rough in the capital.
  • In 2024 (July – September) approximately 798 women and 3,788 men were reported rough sleeping in London (CHAIN Report 2025).
  • The average age of death for people experiencing homelessness is 45 for men and 43 for women (Office of National Statistics).

Volunteer with Us

Help make a difference at our soup kitchen, supporting clients or bring expertise to our Trustee Board. Transition Homeless Project are keen to deliver a vital service that provides nutritious meals and advice to those in need. We are looking for dedicated volunteers to join us in making a positive impact.

How You Can Help

Experienced Cooks: We need volunteers with experience in preparing meals on a large scale. As a cook, you will play a crucial role in ensuring that our service users receive healthy, nourishing food. Food Hygiene Certificate  preferable (training will be provided if necessary).

Kitchen Support: Help supervise the service, assist with cleaning, washing up, and tidying the kitchen after use. Your contribution will ensure the kitchen remains clean and ready for the next service.

Support Workers: Experienced support workers to provided support to clients, previous experience working in the homeless or housing sector is required.

Board Trustees: We are looking for strategic thinking Trustees to join our Board. This is a great opportunity for anyone with knowledge of  poverty prevention and homelessness to make a difference to the community.  You will be involved in the key deliverables of the charity to prevent poverty and homelessness within local communities.

All Trustees will be equally responsible for the proper governance of the charity in accordance with our charitable objects and charity law.  Training opportunities will be available to support you in your role as a trustee.

Many individuals are struggling to feed themselves and obtain basic essentials due to the rising cost of living. Your generous donation can help provide hampers, ensuring individuals that are struggling have essential provisions.

Your contribution ensures someone can enjoy a festive meal bringing some joy during challenging times. With your help, we can provide a little extra care and hope to those in need at Christmas. Together, we can create a holiday season filled with joy and warmth.

Support Workers:

Our experienced support workers provide one-on-one assistance, offering tailored guidance to help individuals navigate their journey towards stability and long-term independence.

Local Rate Number:

We offer a Local Rate Number (available Mon – Fri 9am – 5pm) for individuals to easily access our services and receive the support they need.

Employment Support

After long-term unemployment it can be challenging to find employment. We provide careers workshops that create opportunities for long-term employment.

Workshops Include – Identifying career goals,  CV writing, interview skills, careers coaching.

Support for care leavers

We address the challenges young people experience leaving care, such as poor mental health, potential homelessness and barriers to employment opportunities.

Support for Rough Sleepers

We offer essential services to rough sleepers, providing them with free meals, access to health checks, and the guidance required to transition off the streets.

Domestic abuse survivors

We offer specialist advise and assistance to anyone who has experienced domestic abuse. This  includes a women’s only service that provides a safe space for support, free meals and essential provisions.

At Transition Homeless Project, our mission is to eliminate poverty and prevent homelessness. We understand that early intervention is crucial, which is why we focus on proactive, preventative measures to ensure individuals do not experience financial hardship or homelessness.

Financial and Housing Advice

At Transition Homeless Project, we believe that prevention is key to ending homelessness. We offer professional advice and guidance to help individuals avoid homelessness and regain stability.

Services Include:

Finance and Benefit Support – Expert advisors will assist with budgeting, managing debt, and accessing financial benefits ensuring clients have the resources they need.

  • Housing Advice:

    We provide practical advice on housing options, rights, and responsibilities to help individuals secure and maintain stable accommodation.
  • Temporary Accommodation Assistance:

    We guide individuals through the process of securing temporary housing, ensuring they have a safe place while they work toward long-term solutions.

Through these services, we offer a lifeline to those at risk of homelessness, empowering them with the tools and knowledge to stay safe and secure.

Providing careers workshops to unemployed individuals. Using various tools, resources, and strategies to enhance employability skills, and build confidence.
