About Us
Who Are We
Transition Homeless Project is a non-profit organisation based in North West London. We are committed to transforming the lives of women 18+ experiencing poverty, at risk of eviction, homeless or rough sleeping.
- Preventing homelessness on the streets.
- Providing hot meals and a safe hub.
- Assisting females 18+ facing poverty or unexpected homelessness due to relationship breakdown, domestic abuse, leaving care, job loss, or eviction
We address the causes that push women into homelessness, such as poverty or fleeing abuse. We provide trauma informed care for women, and provide a warm safe space to connect.
Our support is long-term helping women to transition towards a brighter future and thrive in the community. Tailored support is available help women gain skills for employment with our bespoke Careers Programme providing careers support and training.
Women are often invisible ‘hidden homeless,’ they tend to sofa surf, or sleep in hidden places. We primarily focus on women due to the lack of safety and dangers they encounter rough sleeping.

The rising cost of living and energy prices have left many struggling to survive. In London alone:
- Over 187,000 people are homeless (Shelter, December 2024).
- 24.8% of homeless individuals in England and Wales are based in London.
- People aged 25 and under make up 9% of those sleeping rough in the capital
- In 2024 (July – September) approximately 798 women were reported rough sleeping in London
- The average age of death for people experiencing homelessness is 43 for women (Office of National Statistics).

Transition Homeless Project offers a range of services such as a weekly soup kitchen providing hot meals, and dedicated support for females aged 18+. We offer financial advice, housing advice, poverty prevention, plus education and training programmes. Our services are tailored providing one on one support to identify the specific needs of our clients.
