Who Are We

Transition Homeless Project is a non-profit organisation based in North West London, committed to transforming the lives of vulnerable females 18+ at risk of homelessness  or rough sleeping in the community.

Our Mission

Through compassionate care and tailored services, Transition Homeless Project is dedicated to fostering relationships that make a lasting difference in the lives of those in poverty or experiencing homelessness. Helping women move from homelessness to independent living.

We work collaboratively with other local community groups and local councils.

Why We Need Your Help

Homelessness can happen to anyone

The rising cost of living and energy prices have left many struggling to survive. In London alone:

  • Over 187,000 people are homeless (Shelter, December 2024).
  • 24.8% of homeless individuals in England and Wales are based in London.
  • People aged 25 and under make up 9% of those sleeping rough in the capital
  • In 2024 (July – September) approximately 798 women were reported rough sleeping in London.
  • The average age of death for people experiencing homelessness is 43 for women (Office of National Statistics).

Volunteer with Us

Help make a difference at our soup kitchen.
At Transition Homeless Project, our soup kitchen is a vital service that provides nutritious meals to those in need. We are looking for dedicated volunteers to join us in making a positive impact.

How You Can Help

Experienced Cooks: We need volunteers with experience in preparing meals on a large scale. As a cook, you will play a crucial role in ensuring that our service users receive healthy, nourishing food. Food Hygiene Certificate  preferable (training will be provided if necessary).

Kitchen Support: Help supervise the service, assist with cleaning, washing up, and tidying the kitchen after use. Your contribution will ensure the kitchen remains clean and ready for the next service.

Support Workers: Experienced support workers to provided support to clients, previous experience working in the homeless or housing sector is required.

By volunteering with us, you’re helping to rebuild lives and bring hope to individuals facing poverty or homelessness.

Time: 11:00 am – 3:00 pm

Support for food hampers

Many homeless young people and women are struggling to feed themselves and obtain basic essentials due to the rising cost of living. Your generous donation can help provide hampers, ensuring individuals that are struggling have essential supplies.

Making Christmas Special

Your contribution ensures that a young person or single woman can enjoy a festive meal bringing some joy during challenging times. With your help, we can provide a little extra care and hope to those who need it most at Christmas. Together, we can create a holiday season filled with joy and warmth.

Local Rate Number:

We offer a Local Rate Number (available Mon – Fri 9am – 5pm) for individuals to easily access our services and receive the support they need.

Employment Support

Supporting you to find employment

After long-term unemployment it is challenging to enter into employment. We provide strategies to navigate the job market effectively. We provide personal support to find opportunities and transition into long-term employment.

Identifying career goals, personal development skills, CV workshops, careers advise, job application support, interview skills, career coaching.

Preventative Support

Early Intervention and Crisis Support:

We provide timely support to individuals at risk of homelessness, preventing them from losing their homes.

Support for Rough Sleepers

We offer essential services to rough sleepers, providing them with the resources and guidance needed to rebuild their lives and transition off the streets.

Support for Young People and care Leavers

We offer tailored guidance and assistance to young people and care leavers, helping them navigate the challenges they face and build a stable future.

Eliminating Poverty and Preventing Homelessness

At Transition Homeless Project, our mission is to eliminate poverty and prevent homelessness. We understand that early intervention is crucial, which is why we focus on proactive, preventative measures to ensure individuals do not experience financial hardship.

Financial and Housing Advice

At the Transition Homeless Project, we believe that prevention is key to ending homelessness. We offer professional advice and guidance to help individuals and families avoid homelessness and regain stability.

Services Include:

Finance and Benefit Support – Expert advisors will assist with budgeting, managing debt, and accessing financial benefits ensuring individuals have the resources they need.

  • Housing Advice:

    We provide practical advice on housing options, rights, and responsibilities to help people secure and maintain stable accommodation.
  • Temporary Accommodation Assistance:

    We guide individuals in need through the process of accessing temporary housing, ensuring they have a safe place while they work toward long-term solutions.

Through these services, we offer a lifeline to those at risk of homelessness, empowering them with the tools and knowledge to stay safe and secure.
