Who Are We
Transition Homeless Project is a non-profit organisation based in Harrow, West London, committed to transforming the lives of the homeless and vulnerable in our community.
Our Mission
- Preventing Homelessness on the Streets – We focus on early intervention and support to keep individuals and families off the streets and in safe, stable living situations.
- Providing Hot Meals and Support for Independent Living – We offer nourishing meals and the guidance needed to help people regain independence and stability in their lives.
- Assisting Vulnerable Groups – We provide tailored support to young people, and single homeless women, facing unexpected homelessness due to factors such as job loss, domestic abuse, or eviction.
- Supporting Those at Risk of Homelessness – Our services are preventative, designed to address the specific needs of individuals at risk of homelessness, helping them navigate financial or housing difficulties before they escalate.
Through compassionate care and tailored services, Transition Homeless Project is dedicated to fostering relationships that make a lasting difference in the lives of those experiencing homelessness and poverty.
We work collaboratively with other local community groups and the local council.

Why We Need Your Help
Homelessness can happen to anyone
The rising cost of living and energy prices have left many struggling to survive. In London alone:
- Over 187,000 people are homeless (Shelter, December 2024).
- 24.8% of homeless individuals in England and Wales are based in London.
- In 2023, 568 woman were reported rough sleeping in England.
- The average age of death for people experiencing homelessness is 43 for women (Office of National Statistics).
